• Services: Supports provided
• Outcomes: Expected results
• Review dates: When to reassess

Creating a New Plan

1. Go to resident profile
2. Tap "Service Plans"
3. Select "New Plan"
4. Fill in each section
5. Add signatures as required
6. Submit for approval

Annual Reviews

Plans must be reviewed annually:

• System alerts 60 days before due
• Review progress on each goal
• Update objectives as needed
• Document changes and signatures
